2023-24 SCHOOL YEAR ATHLETIC FORMS

There are a number of forms that are either required or that may be needed over the course of a season or school year. All of these forms, except the medical clearance/physical form, will be submitted electronically when you register your student through our online registration system, rSchool.

Students must be registered electronically and have a current physical or alternate year physical form on file to be able to tryout or practice with the team.

Physicals for the 2023-24 school year must be dated 4/1/22 or after to be eligible for the upcoming school year.
